Short answer is, I scaled a cylinder to cover her body, deleted the top and bottom face, cut arm holes out of the cylinder, and extruded the arm holes into sleeves.
From there, just head to sculpt mode, turn on dynamic topology, and sculpt to desired form and fit.
Sorry if I can't be more specific. A better explanation would probably require me to make a 10-15min video.
