Blowjobs
This was probably suggested before or a while ago, but why not add blowjobs? This could just work the same way in how coupling works, but just with the mouth.
And perhaps make it so, when you make the orifice in question oral, the bottom's mouth should try and wrap itself around the penetrator's penis when you close their mouth.
